Cat Adoption Contract

Beyond K9 Advocates (BK9) adoption fees vary upon the age and health status of the cat. Our adoption fee covers expenses such as: vaccinations, spay/neuter, food, transport, shelter fees, and supplies including medical bills for all cats in our organization. We accept cash, cashiers check, Venmo and Zelle. I, _________________________ enter into this Adoption Contract and acknowledge receipt from BK9 of the cat named and described as: ________________(name), Male / Female, ________(age), _______________(breed), _______________(color). I hereby agree to the following terms and conditions pertaining to the pet adopted by me from BK9:

I understand and agree to the following:

1. I understand that I am adopting this cat solely as a household pet. This cat is to be an indoor pet and a secured outdoor pet as per BK9 Terms and Policies located on their website (www.beyondk9.training). If for any reason I cannot keep this cat, I will immediately notify and return the cat to BK9. The cat shall not be given away, re-homed, sold or surrendered to a shelter or a pound at any time for any reason. I understand that if a family member or friend would like to take ownership of this cat they will be required to apply through BK9 for the right to adopt. I also understand that ALL ADOPTION DONATION FEES ARE NON-REFUNDABLE and if said dcat is returned for any reason at any time the adoption fee is considered a tax deductible donation to me. 2. BK9 makes no guarantee about the cats temperament, health, age or mental disposition. BK9 is not responsible for any damages or injuries caused by this animal at any time in any location. I am adopting this pet at my own risk and take full responsibility for him/her at all times. BK9 will not be held responsible for any veterinary care or bills related to my new cat named above immediately after the time of adoption. If my new cat becomes ill within the first seven (7) days after the time of adoption, I agree to call BK9 first before seeing an outside vet. INITIALS _____

3. BK9 may make inquiries about my cat during the lifespan of my dog. I shall promptly respond to any concerns BK9 may have about the status or condition of the cat at any time. INITIALS _____

4. I am to provide food, water, shelter, exercise, mental stimulation and routine medical care for my adopted cat. In the event that I can no longer provide such necessities for my cat I will notify BK9 immediately so that arrangements can be made for the cat to be re-homed or returned to BK9. INITIALS ______

5. I agree to NOT mentally or physically abuse this cat nor let anyone else do so at any time. INITIALS ________

6. I agree to provide a collar and id tag for my new cat prior to bringing them home. INITIALS ______

7. I understand that rabies tags should be attached to my new cats collar at all times and shall be licensed in my township within 7-10 days after adoption. INITIALS _____

8. I understand that BK9 will accept this adopted cat back into their foster care program if I can no longer care for this cat as per their Terms & Policies found on their website (www.beyondk9.training) If major behavioral issues are present and I wish to return my new cat I understand that proof is required that veterinarian evaluation has been done for a minimum of 6 months prior to surrendering my cat. 9. BK9 is not responsible for any major health issues present post adoption and therefore reserves the right to not accept the cat back into their advocacy program if I chose to surrender my cat without addressing any major health issues. INITIALS _____

10. If BK9 agrees to help re-home my cat after all due diligence has been met, I agree that I must be able to hold (foster) the cat until new placement is found unless otherwise agreed upon with BK9. INITIALS ______

11. I agree to notify BK9 immediately if my cat becomes lost, stolen or passes away unexpectedly. If this cat is microchipped I agree to notify the microchip company immediately. I will make all reasonable efforts to recover my cat if he/she becomes missing. I agree to not let this pet roam by itself outside of an enclosed fenced in area and I also agree to never leave this cat unattended in any vehicle that violates my states laws. INITIALS _____

12. I will abide by all local license laws in any location I move my cat to. INITIALS _______

13. I agree to notify BK9 IN ADVANCE if I ever intend to euthanize this cat for any reason. BK9 retains the right to take possession of this cat if they disagree with the reason and/or decision to euthanize it. *Note* This does not include situations where a vet has deemed it necessary to euthanize the cat due to irreversible illness or injury. 14. I have never been subject to legal action for cruelty to animals or neglect to animals. I have never owned an animal which has been confiscated by any animal control, humane officer or humane organization. INITIALS _____

15. I acknowledge that BK9 has the right to reclaim this cat and make appropriate arrangements for it at any time for failure to comply with this adoption agreement, adoption application and all the Terms & Policies listed on their website (www.BeyondK9.training) In the event that BK9 should reclaim this cat, I understand that I am required to forfeit all adoption fees. I understand that I will be responsible for any and all expenses including court costs and attorney fees should I disagree with BK9 and their attempt to reclaim this cat. I understand that this includes fees needed to enforce this contract and to reclaim the cat. INITIALS _____

16. I understand that failure to fulfill any of the above terms and conditions will cause breach of this agreement and will authorize BK9 to reclaim this cat and/or take legal action. This adoption agreement is governed by the laws of New Jersey. Beyond K9 Advocates is a registered non-profit 501c3 organization. All donations not related to an adoption fee are tax deductible with a signed donation receipt. I have read and fully understand this adoption agreement. I agree to fully abide by all terms and conditions stated in this adoption agreement as well as those listed on BK9’s website (www.beyondk9.training)
